Because benefits on most managed care plans vary by employer, we ask that you be aware of your individual benefits, and obtain necessary authorizations from your primary care physician prior to your visit.
Fees, Payment Policy and Credit
We are acutely aware of today's health care costs. We make every effort to keep our fees fair and reasonable.
If we are a participating provider with your insurance plan, we will file a claim with your insurance. You will be responsible for any co-payment or deductible at the time of your visit.
If we are not a participating provider in your plan, our policy is to receive your payment at the time of your visit. As a courtesy to you, we will then file with your insurance company so that you may receive reimbursement from them, according to the terms of your coverage.
There may be some services that are either not covered or limited in coverage. It is important to note that you are ultimately responsible for payment of your account.
